Job summary

Cleveland Clinic Neurological Institute is hiring a Social Worker to support the Epilepsy program across inpatient and outpatient settings. The role emphasizes crisis intervention, resource navigation, counseling, and advocacy within a multidisciplinary team for both pediatric and adult patients.

Shifts are days, full-time, 8:00am–4:30pm with no weekends or holidays. Requirements include a Master’s in Social Work, active LSW licensure, LISW within five years, and at least two years in a hospital

Qualifications

• Master’s Degree in Social Work from a CSWE accredited program.
• Current state licensure as a Licensed Social Worker (LSW).
• State licensure as a Licensed Independent Social Worker (LISW) within five years of hire.
• Two years of experience in a hospital or healthcare setting.

Responsibilities

• Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional, financial and environmental problems.
• Perform psychosocial assessments and provide interventions to assist patients and families (e.g., counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings).
• Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans.
• Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers.
• Maintain a network of resources for consultation and connect patients with community resources.
• Supervise Social Work Interns and assist with training new caregivers.
• Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.
