Job summary

The caregiver will apply crisis intervention, resource navigation, and counseling to empower patients toward stability and improved well-being, with a focus on epilepsy care across complex neurological services.

Qualifications

• Master's Degree in Social Work from CSWE-accredited program.
• Current state licensure as an LSW.
• LISW licensure within five years of hire.
• Two years of experience in a hospital/healthcare setting.

Responsibilities

• Collaborate with healthcare teams to assess and plan interventions for patients and families.
• Conduct psychosocial assessments and provide counseling, education, advocacy, referrals, and crisis intervention.
• Document and communicate treatment recommendations, care coordination, and plans.
