e4health is seeking an Epic Healthy Planet / Value-Based Analytics Consultant to optimize population health and value-based care initiatives across healthcare organizations. You will translate clinical and operational needs into HP-driven analytics, dashboards, and reporting workflows.

The role emphasizes collaboration with clinical, quality, finance, and IT stakeholders to drive improvements in quality, cost, and outcomes using Epic Healthy Planet data.

Qualifications

• Hands-on experience with Epic Healthy Planet and analytics.
• Experience with population health management and quality programs.
• Ability to translate requirements into dashboards and reports.
• Ability to collaborate with clinical and IT stakeholders.

Responsibilities

• Provide functional and analytical expertise for Epic Healthy Planet implementations, optimization, and support.
• Analyze and translate value-based care requirements into Epic Healthy Planet workflows and analytics.
• Develop dashboards and reports for CMS, HEDIS, MIPS, and other programs.
• Identify care gaps and opportunities for clinical intervention using HP analytics.
• Collaborate with clinical teams, quality, finance, and IT leaders to drive value-based outcomes.
• Support data validation, testing, and troubleshooting of Healthy Planet analytics.
