Job description

One of our clients in the government domain is seeking an EPIC-Certified Analyst who will play a vital role in supporting the implementation, optimization, and ongoing maintenance of the Epic Healthy Planet module, which is designed to enhance population health management. This role works closely with clinical, operational, and IT stakeholders to ensure that data-driven, value-based care strategies are effectively supported through Epic. The analyst will help bridge gaps between data insights, care coordination, and clinical workflows, enabling improved health outcomes across patient populations.

Responsibilities:

• Configure, build, and maintain the Epic Health Planet module to support population health initiatives.
• Collaborate with clinical and operational leaders to design workflows that support care management, preventive care, and chronic disease monitoring.
• Analyze population health data and metrics to support the design and implementation of targeted care strategies.
• Develop dashboards and reports to support performance improvement and monitor key quality indicators.
• Assist in upgrades, testing, and changing management for Health Planet and related modules.
• Maintain up-to-date knowledge of Epic upgrades, functionality, and best practices in population health.

Required Skills:

• 2 years Certification in the Epic Healthy Planet module.
• 2 years of experience in configuring, building, and maintaining the Epic Health Planet module to support population health initiatives.
• 2 years of collaborating with clinical and operational leaders to design workflows that support care management, preventive care, and chronic disease monitoring.
• 2 years of analyzing population health data and metrics to support the design and implementation of targeted care strategies

Preferred Skills:

• Epic HIM certification.
• Strong knowledge of HIM operations, workflows, and regulatory requirements.
