Job summary

Qualifications

• Bachelor's degree in Physical Therapy required
• Licensed Physical Therapist in PA required
• Graduate of an accredited physical therapy program required

Responsibilities

• Evaluates patient using diagnostic and prognostic assessments
• Plans individualized physical therapy program for each patient
• Directs and supervises patient in exercises and functional training
• Coordinates physical therapy with interdisciplinary team
• Provides supervision of PTAs and Rehab Aides as required by PA act
• Maintains patient records to support medical necessity and progress
