Job summary

Fresenius Medical Care in Oregon is seeking an entry-level Master’s in Social Work to provide psychosocial services to patients and families, both in-center and at home dialysis when applicable. You will work with the care team to support adjustment, rehabilitation and quality of life.

You will conduct assessments, participate in care planning, provide counseling, education on treatment options and resources, and help patients navigate decisions and insurance needs while adhering to FMCNA

Qualifications

• Masters in Social Work from an accredited program.
• Must have state temporary license or be eligible for licensure with required credentialing.

Responsibilities

• Assess patients' psychosocial status and plan care with the interdisciplinary team.
• Provide counseling and education to patients and families.
• Assist with transplant referrals and access to community resources.
• Support adherence, quality of life, and adjustment to dialysis.
• Document interactions and outcomes per FMCNA policy.
• Participate in quality improvement activities and team meetings.
