Job summary

County Of Maui is seeking an entry-level trainee to learn aging and disability services with training in principles, methods, and procedures. The role involves routine casework under supervision and on-the-job training across the aging network.

Responsibilities include in-home assessments, coordinating with interdisciplinary teams, assisting with eligibility for publicly funded supports, and maintaining participant records.

Qualifications

• Bachelor's degree with at least 12 credit hours in human services, gerontology, psychology, social work, sociology or related behavioral sciences.
• Knowledge of aging and disability services and related laws/regulations.
• Ability to interpret laws, collect and analyze information, and prepare reports.

Responsibilities

• conducts individualized in-home assessments and support planning for cost-effective, outcome-oriented service options.
• works with interdisciplinary team members for effective case coordination.
• provides information about publicly funded long-term supports and assists with eligibility processes.
• assists participants and families in obtaining housing, food, medical care, and medications.
• maintains accurate participant records.
• provides information/referral by phone, mail, email, walk-ins, outreach and presentations.
• complies with federal, state, and local laws and best practices in direct service delivery.
• participates in ongoing training and cross-training with community service providers.
• contributes input to the Division marketing plan.
• performs other duties as assigned.
