Qualifications

Responsibilities

• Conduct client assessments identifying history, current presentation, problem areas, and treatment needs.
• Conduct ongoing assessments of services required by the program.
• Prepare and participate in treatment planning and progress reviews.
• Conduct group, individual or family sessions using evidence-based modalities.
• Provide client and family education on substance abuse/psychiatric issues.
• Ensure timely, clear documentation of care and incidents.
• Provide crisis intervention and seek consultation as appropriate.
• Plan and document aftercare for assigned clients.
• Liaise with insurance carriers as requested.
• Provide case management ensuring quality care.
• Conduct educational workshops in hospital and community.
• Serve as liaison with self-help groups and EAP professionals.
