Job summary

ADAPT Community Network in New York seeks an Entitlements Coordinator to manage federal, state, and local public benefits for people served. You will work under the Senior Entitlements Coordinator and collaborate with Residential Directors, Program Leadership, Finance, and Care Management.

The role requires strong organizational and communication skills, experience with Medicaid, SSI, SSDI, and related programs, and a commitment to HIPAA and confidentiality.

Qualifications

• Bachelor's degree in Social Work, Human Services, Public Administration, Healthcare Administration, or a related field required.
• Minimum one year of experience administering or coordinating public benefits or entitlement programs.
• Working knowledge of Social Security, Medicaid, SNAP, and related entitlement and public benefit programs.
• Experience working with OPWDD-funded services and people supported with developmental disabilities strongly preferred.
• Strong organizational skills with the ability to manage multiple priorities and deadlines.
• Excellent verbal and written communication skills.
• Ability to maintain confidentiality and exercise sound judgment.

Responsibilities

• Coordinate the application, verification, renewal, recertification, and ongoing maintenance of public benefits, including SSI, SSDI, Medicaid, MSP, SSP, SNAP, and other applicable Federal, State, and local benefit programs.
• Verify benefit eligibility for new admissions and support continuity of benefits throughout service delivery.
• Prepare, review, and submit required documentation to Federal, State, and local agencies in accordance with established deadlines.
• Monitor pending applications and follow up with government agencies to facilitate timely benefit determinations.
• Track renewals, recertifications, and required documentation to help prevent interruptions in benefits and services.
