Kalihi-Palama Health Center in Honolulu, HI seeks a program support specialist to assist uninsured/underinsured clients with eligibility for Quest, TANF, Medicaid/CHIP, and other DHHS programs. Qualifikationen

• High School Diploma or GED required.
• Post-secondary education or Bachelor's Degree preferred.
• Knowledge of community resources and social services.

Aufgaben

• Assist uninsured/underinsured clients with eligibility for Quest, TANF, Medicaid/QUEST and CHIP, and other DHHS programs.
• Provide accurate enrollment assistance and follow-up for eligible programs.
• Screen and refer clients to community social services based on SDOH.
• Refer clients to providers offering sliding fee scales or free care when appropriate.
• Report timely outcomes and outreach activities to supervisor/HPCA.
