Job summary

360 Management LLC in Lancaster County, Pennsylvania, seeks an Enrollment Case Manager to conduct in-home assessments, process applications, and guide eligibility determinations for the PA IEB program. The role blends field visits with remote administrative tasks to enroll individuals and families in care services.

The ECM works Monday–Friday with travel across PA, using COMPASS tools and data tracking in Excel to ensure accurate, timely enrollments.

Qualifications

• Bachelor's degree with at least 12 credits in Sociology, Psychology, Social Welfare, or any Human Services or Social Science field.
• Experience in case management, social services, or a related field.
• Ability to work independently while managing multiple priorities and deadlines.
• Strong organizational, documentation, and communication skills.
• Proficiency with Microsoft Excel, Word, and Teams.
• Reliable transportation and valid driver’s license.
• Ability to adapt quickly to changing processes and requirements.
• Familiarity with COMPASS or similar eligibility systems
• Standout: bilingual in Spanish/English.

About the Role:

Physical Requirements

• Frequent Travel throughout assigned areas of Pennsylvania.
• Ability to sit for extended periods while maintaining focus during documentation or communication.
• Regular mobility for entering homes, shelters, and facilities.
• Ability to lift up to 25 lbs. (computers, case files, and materials).
• Adequate vison and hearing for driving and communication.
• Capacity to adapt to a range of home and outdoor environments.

Pay Range:

$20.50 - $21.50 with mileage and expenses fully reimbursed.

After successfully completing a contingency period of at least 120 days with our company, eligible employees will have access to an excellent benefits package, including full insurance and other benefits effective the first of the month following the end of the probationary period. In addition, employees receive paid time off for Pennsylvania state holidays from day one, provided they work their scheduled hours before and after the holiday.
