Job summary

Continental Care and Rehabilitation in Montana seeks an Activity Assistant to support residents with engaging recreation and social programs. You will help plan, develop, organize, implement, and evaluate activities in line with residents' plans and supervisor directions.

The role requires a positive attitude, patience, dependability, energy, self-motivation, and good organization. Experience is welcome but not required, and you will be trained to coordinate transportation and link residents

Responsibilities

Assist in planning, developing, organizing, implementing, and evaluating activity programs.
Observe resident attendance, mood, behavior, and involvement; document progress.
Participate in discharge planning and activity care plans.
Interview residents or family members for activity information.
Collaborate with other departments to coordinate activities.
Arrange transportation for field trips when necessary.
Refer residents to appropriate social services when needed.
Keep residents engaged and involve family in planning goals.
