Job summary

Forest Hills Rehabilitation and Healthcare Center is seeking a Full-Time Activities Aide for the 12:30p-8:30p shift. You will assist in planning, developing, organizing, and implementing the activity program under the supervision of the Activities Director.

Responsibilities include meeting with the Activities Director, coordinating with other departments, organizing in-service trainings, arranging field trips, maintaining records, and supporting the volunteer program.

Qualifications

• High school diploma or equivalent required.
• Prior experience in activities program preferred.
• Excellent communication and organizational skills.

Responsibilities

• Assist in planning, developing, organizing, and implementing the activities program.
• Meet with the Activities Director for planning activities.
• Develop and maintain a good working rapport with other departments within the facility and network with outside community agencies.
• Assist in the development of, and participation in, regularly scheduled in-service training programs.
• Assist in the arrangement and escort of residents with field trips whenever necessary
• Assist in the recording and maintaining records of resident’s participation in activities
• Maintain CONFIDENTIALITY of all pertinent resident care information to assure residents rights are protected.
• Assist with the maintenance of the Volunteer Program.
• Report any changes in the resident’s condition immediately to the Nursing Department.
• Reports all incidents/accidents immediately.
• Assist in the scheduling of outside activities and groups for the residents.
• Recommend to the Activities Director the equipment and supply needs of the department.
• Distribute mail/periodicals to residents.
• Assist in assessing resident upon admission and chart resident’s progress.
• Prepare and post the Reality Orientation Board regularly.
• Performs other duties as assigned.
