Forest Hills Center is seeking an Activity Aide to assist in planning, implementing, and evaluating activity programs for residents. The role involves observing mood and participation, documenting progress, and coordinating with other departments to support resident well‑being.

The position includes developing activity plans, interviewing residents and families, involving them in goal setting, and referring to social services when necessary. Driving residents to appointments may be required.

Responsabilités

• Assist in planning, developing, organizing, implementing, and evaluating the activity programs.
• Observe resident attendance, mood, behavior, and degree of involvement, and document progress.
• Participate in community planning related to the interests of the facility and residents.
• Develop and implement activity care plans and resident assessments.
• Interview residents or family members to obtain activity information.
• Involve residents and families in planning activity programs and goals.
• Refer residents/families to appropriate social services when needed.
• Coordinate activities with other departments as necessary.
• Assist in maintaining quality assurance for the activity department.
• Ensure charted activity progress notes are informative and reflect resident response.
• Perform administrative requirements and submit reports to the Activity Director.
• Drive clients/residents on our bus to pick them up, drop them off, or to appointments.
