Job summary

Equus Workforce Solutions seeks a dedicated outreach and engagement professional to support the WeCARE program in coordinating clinical assessments and client services. You will review health-related documents, explain services, and help clients collect needed materials for assessments to maximize benefits.

The role includes scheduling, data entry in MIS, and assisting in resume development and job applications, with focus on rapid client engagement and successful outcomes.

Qualifications

• Bachelor's Degree in social services or related field.
• Four years direct customer service experience in a clinical or social service environment.
• Experience in a fast-paced, team environment that is performance driven.
• Knowledge of laws and regulatory requirements related to program responsibilities.
• Experience with publicly-funded programs; TANF experience preferable.

Responsibilities

• Outreach to customers referred by HRA to schedule clinical assessments and engage in activities.
• Assist in reviewing medical documentation and collecting documents for assessments.
• Explain program processes and benefits, provide written materials, answer questions.
• Schedule external evaluations and coordinate activities with customers.
• Maintain records and generate real-time reports for management.
• Assist with resume development, job applications, and interview skills.
• Support care managers with scheduling and calendar management.
• Prepare for customer workshops and maintain welcoming work areas.
