Job summary

East Bay Community Action Program (EBCAP) seeks a Smart Optimal Solutions Coach to provide one-on-one assessments and ongoing case management for LIHEAP participants. You will help clients set goals, improve financial stability, and connect with community resources and workforce opportunities.

The role links EBCAP with partners and participants, supporting outreach, workshops, and data tracking to ensure effective service delivery and program improvement.

Qualifications

• Bachelor's Degree in Human Services or related field preferred.
• High School Diploma or GED with equivalent relevant experience will be considered.
• Experience in case management, community services, social services, workforce development, financial education, or related fields is valued.

Responsibilities

• Conduct comprehensive assessments and provide individualized case management to eligible households.
• Support clients with energy conservation education, financial literacy guidance, and workforce development referrals.
• Build relationships with clients using a strengths-based, client-centered approach.
• Coordinate referrals with internal programs and external partners.
• Assist clients with LIHEAP eligibility and related service applications.
• Monitor progress, follow-up, and update service plans as goals evolve.
• Maintain accurate case documentation and data entry.
