Responsibilities

• Assesses, plans, implements, evaluates and documents nursing care of patients per policy and nursing standards.
• Formulates and revises the plan of care including discharge planning and communication with the multidisciplinary team.
• Provides direct patient care, coordinates activities, delegates tasks, and ensures continuity of care across shifts.
