Qualifications

• Licensed Registered Nurse (RN) in good standing.
• Experience applying nursing process and evidence-based practice.

Responsibilities

• Deliver patient care using the nursing process and Relationship Centered Care principles.
• Assess patient needs and apply critical thinking in care management.
• Collaborate with the interdisciplinary team in developing and evaluating the care plan.
• Prioritize patient care activities including teaching, rounding, coaching and discharge planning.
• Educate patients and caregivers about plan of care and transition of care.
