Job summary

Griffin Health in Derby, CT seeks an Advanced Practice Provider (APP) for the Endocrinology service. In this outpatient role you will perform comprehensive evaluations, diagnose and manage diabetes and endocrine disorders, and develop individualized treatment plans while educating patients on medications and self-care.

The ideal candidate has NP/PA training and at least 3 years in endocrinology, with active licensure and strong clinical skills; board certification and gestational diabetes

Qualifications

• Graduate of an accredited NP or PA program.
• Minimum 3 years of endocrinology experience.
• Current licensure in the state of practice.
• Board certification preferred (AANP/ANCC/NCCPA).
• Experience gestational diabetes or women's metabolic health preferred.
• Familiarity with insulin pumps and CGMs.
• Strong communication and patient education skills.
• Team-oriented with collaboration across disciplines.

Responsibilities

• Perform comprehensive evaluations, including history, physicals, and diagnostics for endocrine conditions.
• Diagnose and manage diabetes and other endocrine disorders with individualized treatment plans.
• Provide patient education on disease management, medications, and self-care.
• Collaborate with physicians, educators and care managers; coordinate transitions.
• Contribute to quality initiatives and documentation in the EHR.
• Potential involvement in inpatient consults and program development in diabetes care.
