Job summary

Epic Care is seeking a dedicated Advanced Practice Practitioner (NP/PA) to work with an Endocrinologist at our San Leandro and Dublin clinics. You will provide follow-up care for endocrine disorders, including diabetes, and manage medications for glucose, BP, and cholesterol while collaborating with the care team.

Ideal candidates hold an NP or PA credential with a Master of Science in Nursing and California licensure, plus specialty certification.

Qualifications

Master of Science Degree in Nursing from an accredited NP program; specialty certification required.
RN-BSN with California licensure to practice as NP/PA.
Current California licensure to practice medicine or mid-level provider.

Responsibilities

Provide follow-up and ongoing management care to patients with endocrine disorders, including diabetes.
Medication management for glucose, blood pressure, and cholesterol control.
Ongoing communication with referring clinicians; coordinate care with physicians and healthcare team.
Develop expertise in diabetes and endocrine disorder management.
Manage an assigned patient caseload with physicians and healthcare team.
Coordinate patient care across settings to optimize health outcomes.
