Job summary

Sevita in New Jersey is seeking a Direct Support Professional to provide person-centered assistance in residential and community settings, helping individuals with daily living activities, health maintenance, and socialization per their plans.

You will document progress, support transportation, protect rights and confidentiality, and collaborate with a team to maintain a safe, respectful environment and promote skill development.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service/program plans, including socialization and development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately document progress and activity; review records and logs to stay abreast of changes in service plans; maintains confidentiality.
• Follow safety policies, report incidents, and maintain professional boundaries.

Education

High school diploma or equivalent

Six months of experience in human services

Must be 18 years of age

Working knowledge of computers
