Job summary

Sevita is seeking a Direct Support Professional in New Jersey to support individuals in residential settings. You will assist with daily living tasks, socialization, health maintenance, and transportation while following individualized service plans.

Responsibilities include documenting progress, coordinating with nurses and supervisors, and maintaining confidentiality. A high school diploma is preferred and six months of experience in human services is desirable; a valid driver’s license is

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately documents progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.
• Communicates with nurse or supervisor; follows Sevita policies; reports incidents and concerns as required.
