Job summary

Prestera Center for Mental Health Services in Wisconsin is seeking a field-based job coach to support individuals with disabilities in achieving job readiness and successful employment. You will provide one-on-one coaching, conduct skills assessments, and develop individualized plans to help clients secure and maintain placements.

Responsibilities include coordinating with worksites, arranging transportation when needed, and maintaining records in program guidelines.

Qualifications

• High school diploma or GED required.
• Knowledge of mental health, substance use disorder, and disability populations and supported employment practices.
• Valid state-issued driver’s license.
• Proficiency with MS Word, Excel, PowerPoint and electronic scheduling/EMR systems.
• Ability to communicate and coordinate with employers, partners, and agencies.

Responsibilities

• Assists in developing independence and successful employment.
• Provides one-on-one job coaching and support to individuals with barriers to employment.
• Conducts skills assessments to determine job fit.
• Develops individualized program plans to support job readiness and success.
• Provides ongoing follow-up and support to ensure job satisfaction and retention.
• Coordinates daily workflow and staffing at worksites; ensures coverage per shift.
• Acts as liaison among Prestera, worksites, job coaches, and partner agencies.
