Qualifications

• Associate’s degree in Social Services or related field with two years of experience in community or social services delivery.
• Knowledge of local, state, and federal public benefits for eligible residents.
• Familiarity with housing authorities, HUD reporting, and service coordination.

Responsibilities

• Coordinate with community service providers to tailor services for residents.
• Interview and assess at-risk residents to identify needs and connect with resources.
• Develop case action plans to assist individuals and families in improving social functioning.
• Maintain regular contact with residents and collaborate with facility staff.
• Document resident progress and prepare activity reports for HUD compliance.
• Assist with scheduling events such as workshops and trainings.
