Job summary

Sunrise Community, Inc. is seekingDedicated Direct Support professionals to provide assistance and supervision to individuals with developmental disabilities, enabling them to live valued lives in the community.

Responsibilities include implementing support plans, monitoring health and safety, and documenting progress while maintaining a positive, empowering environment. A valid driver's license and minimum 3 years of driving experience are required.

Qualifications

• Must possess a valid Driver's License with all endorsements and have at least 3 years of driving experience.
• Must possess basic computer skills and be able to use standard software.
• Experience in the human service field is preferred depending on location.

Responsibilities

• Identifies and reports suspected abuse or violations of an individual’s rights.
• Provides support, assistance and supervision to meet the needs and goals of individuals per their plans.
• Ensures training, behavior and support programs are implemented and documented per regulatory requirements.
• Maintains health, safety and well-being of individuals and fosters independence and choice.
• Follows positioning schedules and uses equipment per plan.
