Job summary

TRI-KO INC is seeking a Case Manager to assist individuals and their networks in identifying and coordinating services to promote independence and integration. You will manage case records, conduct home visits, and advocate for clients under KDADS guidelines.

Responsibilities include coordinating PCSP meetings, documenting monthly activities, and collaborating with staff across employment, education, habilitation and residential plans to support individuals in least restrictive environments.

Qualifications

• Bachelor's degree in human services or related field required.
• Minimum 6 months experience with individuals with ID/DD.
• Valid driver's license and ability to be covered by corporate vehicle insurance.
• Strong organizational and documentation skills.

Responsibilities

• Manage assigned individual case records and keep documentation up-to-date.
• Conduct home visits, crisis intervention and referrals to community agencies.
• Coordinate PCSP meetings and advocate for individuals served.
• Support employment, education, habilitation, and residential plans within least restrictive settings.
