Responsibilities

• Assess client needs and functioning to determine appropriate services.
• Develop and monitor care plans using the online referral system.
• Provide information, referrals, and linkage to services.
• Advocate for clients to navigate social services and government systems.
• Provide counseling on aging, health, finances, bereavement, and grief.
• Monitor services via telephone and home visits; conduct public presentations.
• Prepare reports and maintain confidential records.
