Job summary

Creative Solutions in Healthcare - South is seeking a full-time Physical Therapy Assistant (PTA) to join our in-house rehab team. You will deliver patient-centered PT interventions under a PT, help residents improve mobility and quality of life, and document outcomes in Net Health.

Ideal candidates hold an active Texas PTA license, have SNF or long-term care experience, and communicate well with patients, families, and the care team.

Qualifications

• Active and valid Texas Physical Therapy Assistant license.
• In good standing with regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication and teamwork skills.

Responsibilities

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Provide skilled physical therapy services and interventions under the supervision of a Physical Therapist.
• Respond to requests for service by relaying information and referrals to the Physical Therapist.
• Implement and modify patients' plans of care under PT supervision.
• Measure and record patients' motor function, strength, and muscle performance.
• Monitor patient response to treatment and adjust approaches with the interdisciplinary team.
• Provide education to patients, family members, and caregivers.
• Ensure documentation is thorough and timely per policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
