Association for Individual Development in Batavia, IL is seeking a Case Manager Mental Health Professional IM+CANS Specialist to support adults with persistent mental illness and co-occurring substance use disorders within BH Outpatient/PSR and Supported Programs.

You will perform assessments, IM+CANS development, counseling, case management, and coordinate with a team of providers to meet client needs while maintaining timely documentation and adherence to program funding rules.

Qualifications

• Bachelor's degree in human services required.
• IM+CANS certification or ability to obtain within 30 days.
• Experience in behavioral health and case management preferred.

Responsibilities

• Attend scheduled work hours and provide IM+CANS reassessment services.
• Complete clinical assessments per program requirements and funding rules.
• Provide case management services and timely documentation.
• Maintain productivity and meet service hour standards.
• Collaborate with agencies and stakeholders to improve services.
