Qualifications

• Current, active LPN/LVN license in Tennessee.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs to promote comfort, dignity and safety.
• Supervise and direct nursing assistants during the shift per policy and regulations.
• Monitor and observe residents’ status; notify physician or family of changes.
• Develop, implement, and review individualized resident care plans with the team.
• Complete accurate clinical documentation per policy and procedure.
• Coordinate resident admissions, transfers, and discharges per procedures.
• Participate in quality assurance activities, staff education, and audits.
