Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to deliver in-home SLP services, evaluating and treating patients to improve speech, language, swallowing and communication.

You will develop care plans, perform assessments, provide training to patients and families, and coordinate with the clinical team while maintaining documentation and ensuring patient confidentiality.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA) required.
• Minimum six months experience as a speech language pathologist.
• Current and unrestricted state license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation required.

Responsibilities

• Evaluate and provide speech/language pathology services in the home or facility.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, language, voice, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapies including audiologic screening.
• Observe and report changes to supervisor/physician.
• Provide instruction in alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff.
• Maintain documentation and prepare reports as required by policy.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and share information only with authorized recipients.
