Job summary

CenterWell Home Health, part of Humana, is seeking a Speech-Language Pathologist to deliver in-home services. You will evaluate, treat, and coordinate care to help patients regain communication and swallowing abilities, working under the Clinical Manager.

The role requires a current SLP license, CPR, and a valid driver’s license with reliable transportation. Pay per visit ranges, with emphasis on rehabilitative outcomes and home-based patient support.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent and minimize effects.
• Administer and interpret diagnostic tests and therapeutic treatments including audio logic screening.
• Observe, record and report changes in the patient's condition to supervisor/physician.
• Provide instruction and training to patients in alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and staff.
• Maintain and submit documentation as required; prepare reports and care plans.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and adhere to policies.

Skills

Six months experience as speech path
