Qualifications

• Must have minimum 6 months’ RN experience.
• Experience caring for seniors preferred.
• Basic computer skills including Microsoft Office and web apps.
• EMR experience preferred.

Responsibilities

• Develop and implement resident service plans with the care team.
• Monitor resident condition and communicate changes to physicians and families.
• Oversee home care team delivering nursing care per plan.
• Participate in care conferences and staff training.
