Job summary

HCA Midwest Healthcare at Home is seeking a Home Health Registered Nurse to provide skilled nursing in patients' homes, collaborating with the interdisciplinary team to achieve patient goals and deliver high-quality, compassionate care.

You will perform patient assessments, complete OASIS and visit documentation, supervise aides and LVNs/LPNs, and participate in on-call rotations to ensure continuity of care and rapid response.

Qualifications

• Basic Cardiac Life Support credentials within 30 days of start date.
• RN license and valid driver license (DL).
• Associate degree or RN diploma in nursing.

Responsibilities

• Assess home care patients' physical, psychosocial and environmental needs.
• Complete OASIS, assessments and visit paperwork per agency policy.
• Document findings and coordinate with physician and interdisciplinary team.
• Supervise home health aides and LVNs/LPNs to ensure quality.
• Participate in on-call rotation and emergency calls per policy.
