Job summary

CenterWell Home Health is seeking a Home Health Physical Therapy Assistant to support patients at home under the supervision of a Physical Therapist. You will help restore strength, mobility and independence, enabling patients to regain function and quality of life in their familiar surroundings.

The role requires an Associate degree in Physical Therapy, current licensure, CPR, valid driver's license, and excellent teamwork. Competitive pay and comprehensive benefits are offered.

Qualifications

• Associate degree in Physical Therapy from an American Physical Therapy Association.
• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population.
• Read, write, and speak fluent English.

Responsibilities

• Deliver and monitor the physical therapy plan for home-health patients as prescribed by the Physical Therapist.
• Administer non-complex manual therapies, therapeutic massage, and modalities per protocol.
• Document client records and submit weekly clinical documentation.
• Attend educational meetings and coordinate department in-services.
• Report equipment or supply issues to supervisor.
• Apply infection control and universal precautions in patient care.
