Job summary

Bella Terra Home Health & Hospice is seeking a skilled Physical Therapist (PT) to join our Home Health team in California. The PT will deliver high-quality therapy per physician plans of care and coordinate with interdisciplinary teammates to support patient recovery and functional goals.

Qualifications

• Active California Physical Therapy license required.
• Strong verbal and written communication skills.
• Strong organization skills.
• Current CPR certification.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Provide skilled PT services per physician's plan of care.
• Conduct ongoing assessments and update care plans with the team.
• Educate patients and families on therapy goals and home exercises.
• Document visit notes and progress summaries accurately.
• Coordinate with physicians and care team on patient progress.
• Instruct caregivers on techniques and safe home care.
• Train staff on prosthetics and assistive devices as needed.
• Identify additional home health service needs and coordinate referrals.
• Participate in discharge planning and follow-up recommendations.
• Provide in-service education to agency staff.
• Supervise PTAs per state guidelines.
