Guardian Angel Home Care in California is seeking licensed Physical Therapists (PTs) to deliver high-quality home health services. As a Home Health PT, you will evaluate patients in their homes, develop treatment plans, and guide recovery to maximize independence and reduce hospital readmissions.

New graduates and experienced PTs are encouraged to apply. This role offers flexible scheduling options, autonomy in patient care, and strong clinical support across 20 offices nationwide.

Qualifications

• Active PT license in the state of practice.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• CPR/BLS certification required.
• Valid driver’s license and reliable transportation.

Responsabilités

• Perform comprehensive patient evaluations in the home.
• Develop individualized treatment plans based on physician orders.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on home exercise programs.
• Implement fall prevention and safety strategies.
• Document patient progress in EMR systems.
• Communicate with physicians, nurses, and interdisciplinary care teams.
• Adjust treatment plans based on patient progress.
