Job summary

Bella Terra Home Health and Hospice is seeking a Part-Time Physical Therapist (PT) to deliver high-quality home-based therapy under a physician’s plan of care. You will assess patients, develop and update care plans, and collaborate with families and an interdisciplinary team.

The role requires a CA license, CPR certification, and a valid driver’s license with a reliable vehicle. You’ll work across the Beaumont/Banning/Redlands/Moreno Valley coverage area, enjoying autonomy and team support.

Qualifications

• Active CA Physical Therapy License required.
• CPR certification required.
• Must have a valid driver’s license and reliable vehicle.

Responsibilities

• Provide skilled physical therapy services based on the written physician’s plan of care.
• Conduct comprehensive assessments and collaborate with patient, caregiver, and team to update the plan of care.
• Educate patients, caregivers, and families on goals, safety, and home exercises.
• Prepare accurate and timely clinical documentation, including visit notes and progress summaries.
• Communicate with the physician and healthcare team regarding patient progress and changes in condition.
• Initiate and implement the physical therapy program, instructing caregivers on safe techniques.
• Participate in case conferences to review goals, progress, and care plans.
• Train patients in the proper use of prosthetics and assistive devices.
• Identify needs for additional home health services and coordinate referrals.
• Participate in discharge planning and recommendations for continued care.
• Provide in-service education and staff training as needed.
• Engage in peer consultation and quality improvement processes.
• Supervise Physical Therapist Assistants (PTAs) per guidelines.
