PARS Therapy in Carrollton, TX is seeking a compassionate Physical Therapist to join our home health team. You will evaluate patients at their homes, develop individualized plans, and help them regain movement and independence.

Qualifications

• Active Texas PT license.
• Professional liability insurance coverage.
• Graduate degree or higher in Physical Therapy.
• Experience in home health or rehabilitative settings preferred.
• Strong ability to adapt care approaches to diverse patients.
• Up-to-date knowledge of evidence-based treatment techniques.
• Excellent communication and professional demeanor.

Responsibilities

• Evaluate patients' physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust plans as necessary to achieve optimal outcomes.
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with interdisciplinary team members including physicians, nurses, and other therapists.
• Conduct home safety assessments and make recommendations to prevent falls or re-injury.
• Deliver compassionate care while empowering patients to achieve greater independence.
