Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver in-home therapy under a supervising Physical Therapist. You will help geriatric patients improve function, mobility and independence using prescribed PT plans and safe home-based practices.

The role requires a state PT/PTA license, valid driver’s license with insurance, CPR, teamwork, and fluent English; 40 hours weekly; base pay aligns with market, with full Humana benefits.

Qualifications

• Current state license to practice Physical Therapy
• Valid driver’s license and reliable transportation
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Geriatric population knowledge
• Fluent English communication

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes, under supervision of Physical Therapist
• Perform non-complex manual therapy, exercises, and modalities as prescribed
• Document client records and submit reports weekly
• Attend educational meetings and department in-services
• Notify PT or Branch Director of equipment or staff changes
• Apply infection control and universal precautions in patient care

Education

Current state license to practice Physical Therapy (PT) or PTA license

Valid driver’s license and reliable transportation

Current CPR certification

Experience collaborating with healthcare professionals

Fluent English (read, write, speak)
