Job summary

Preferred Care Home Health in Fort Myers, FL is seeking a licensed Physical Therapist to join our Home Health Team. As a Home Health PT, you will evaluate patients, develop individualized treatment plans, and help patients regain independence in the comfort of their homes.

The role also includes supervising PTAs, completing documentation, and participating in care coordination and quality initiatives. Florida license, CPR, and at least one year of PT experience are required; home health

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Florida Physical Therapist license in good standing.
• Minimum one year of physical therapy experience.
• CPR certification.
• Valid Florida driver's license and reliable transportation.
• Strong communication and patient education skills.
• Home health experience preferred but not required.

Responsibilities

• Evaluating patients and developing individualized treatment plans based on physician orders.
• Providing skilled physical therapy services to improve strength, mobility, balance, endurance, and functional independence.
• Monitoring patient progress and modifying treatment plans as appropriate.
• Communicating changes in patient condition to the physician and interdisciplinary care team.
• Educating patients and caregivers on home exercise programs, safety techniques, adaptive equipment, and fall prevention.
• Supervising Physical Therapist Assistants (PTAs) in collaboration with the Director of Rehabilitation.
• Completing accurate and timely clinical documentation.
• Participating in case conferences, care coordination, quality improvement activities, and continuing education.
