Job summary

Enhabit Home Health & Hospice seeks a dedicated Physical Therapist to provide skilled home health services across patients in the community. You will evaluate and treat individuals with physical impairments, applying modalities to improve mobility and independence while educating families and staff on the care plan.

The role emphasizes driving to patient homes, adherence to physician orders, and collaboration with the Branch Director to ensure quality outcomes in the home setting.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Improve mobility and function, independence in self-care and daily living activities.
• Provide skilled care under physician orders and established plan of care.
• Educate family and health care team members to support home care.
