Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to evaluate and treat patients in the home setting. You will use physical modalities to improve mobility and functional independence, and work under physician orders with the branch director’s supervision.

Responsibilities include implementing plans of care, documenting progress, and coordinating with families and care teams to maximize patient outcomes in a home care environment.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must have an active license in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Improve mobility and function, independence in self-care, and daily living skills.
• Provide skilled care under physician orders and plan of care, with supervision from the Branch Director.
