Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health to assess patients, develop treatment plans, and provide interventions to improve mobility and overall well-being. You will work as part of a care team to deliver outcomes that align with the plan of care and state regulations.

The role requires an active physical therapy license in the state of practice, a valid driver's license, and reliable transportation.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws and the plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain and restore function.
• Instructs patient and family/caregivers regarding plan of care and progress.
• Visits patients per Plan of Treatment and submits progress notes.
• Develops PT plan of care within state guidelines.
• Reports outcomes of evaluation and anticipated frequency of care.
