Choice Health at Home in Athens, TX is seeking a dedicated Physical Therapist to join our home health team. You will assess mobility, design personalized treatment plans, and monitor progress to help patients regain independence in familiar surroundings.

Responsibilities include educating patients and caregivers, collaborating with the care team, and ensuring accurate documentation. A state license, CPR, and a valid driver's license are required; home health experience is preferred.

Qualifikationen

• Graduation from an accredited physical therapy program and successful licensure exam.
• Current, valid Physical Therapy license.
• Current CPR certification.
• Valid driver’s license and auto insurance.
• One year of home health experience preferred.
• HomeCare HomeBase experience preferred.

Aufgaben

• Evaluate patients’ mobility, strength, balance, and functional abilities to identify therapy needs.
• Develop and implement individualized treatment plans that promote recovery and independence.
• Monitor patient progress and adjust treatment plans to support the best possible outcomes.
• Educate patients and caregivers on exercises, safe movement, and strategies to support continued progress.
• Collaborate with the healthcare team to ensure coordinated, quality patient care.
• Follow infection control practices and agency policies.
• Complete accurate clinical documentation within company guidelines.
