Job summary

Elite Home Health, part of the LHC Group, is seeking a PRN Occupational Therapist to assess and address patients’ functional needs in their homes. You will apply evidence-based methods, work with a care team, and document outcomes to support CMS requirements while delivering compassionate care.

State OT license and a valid driver’s license are required; CPR is preferred. This flexible PRN role offers meaningful patient impact and growth opportunities within a national post-acute care family.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification.

Responsibilities

• Assessment and evaluation of patient care needs related to functional status and activities of daily living.
• Develops and implements therapy plans of care in collaboration with the health care team.
• Documents patient progress and communicates changes to the physician and nurse supervisor.
• Conducts home visits and adapts therapy to various home environments.
• Demonstrates professional communication and organizational skills.
