Choice Health at Home is seeking a Home Health Field RN (PRN) in Grapevine, TX to deliver patient-centered care in patients' homes. You will assess, plan, implement, and evaluate care, collaborating with interdisciplinary teams to support recovery and independence.

The role requires an active Texas RN license, CPR, a valid driver’s license, auto insurance, and at least 2 years of Home Health experience; HomeCare HomeBase experience is preferred.

Qualifikationen

• Current RN license (Texas) in good standing.
• Valid driver’s license and reliable transportation.
• CPR certification.
• Auto insurance valid.
• Minimum 2 years' Home Health RN experience preferred.
• HomeCare HomeBase experience preferred.

Aufgaben

• Perform initial and ongoing home health assessments and develop plans of care.
• Coordinate admissions, re-certifications, and levels of care.
• Educate patients and families on self-care, nutrition, and care needs.
• Provide clinical directions to HH Aide and LPN/LVN to ensure quality and continuity of service.
• Document progress and communicate changes to physicians.
• Infection control and safety precautions in patient care activities.
• Monitor cases for third party payor requirements.
