Job summary

Choice Health At Home in Palestine, TX is seeking a Home Health Registered Nurse to deliver patient-centered care in patients' homes. You will perform comprehensive assessments, plan and implement care, and educate patients and families to support recovery and chronic disease management.

This role requires an active RN license, CPR, a valid driver’s license, auto insurance, and 2+ years of home health experience; HomeCare HomeBase experience is a plus.

Qualifications

• Current RN license is required.
• CPR certification is required.
• Valid driver’s license is required.
• Valid auto insurance is required.
• At least 2 years of Home Health RN experience is preferred.
• HomeCare HomeBase experience is preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients in their homes.
• Develop, implement, and document plans of care tailored to patient needs.
• Educate patients and families on self-care, nutrition, and disease management.
• Coordinate with physicians and care teams to adjust plans as needed.
• Provide clinical direction to Home Health Aide and LPN/LVN as required.
• Document progress notes and communicate changes in patient status.
