Job summary

Career Links, LLC is seeking a dedicated Occupational Therapist (OT) to provide services in Skilled Nursing Facility (SNF) and Home Health settings across Iowa. The role focuses on helping patients regain independence and improve daily living skills through patient-centered care.

The OT will evaluate and treat patients with physical, cognitive, developmental, or psychosocial limitations, collaborating with interdisciplinary teams to maximize functional outcomes.

Qualifications

• Graduate of an accredited OT program with a Bachelor's, Master's, or Doctoral degree in Occupational Therapy.
• Hold Iowa OT license issued by the Iowa Board of Physical and Occupational Therapy.
• CPR/BLS certification or ability to obtain prior to start.
• Valid driver''s license and reliable transportation for home health travel.
• Strong clinical, communication, and documentation skills.
• Experience in SNF, Rehabilitation, LTC, or Home Health preferred but not required.
• New graduates are encouraged to apply.

Responsibilities

• Conduct comprehensive occupational therapy evaluations and assessments.
• Develop individualized treatment plans based on patient needs, goals, and physician orders.
• Provide therapeutic interventions to improve functional mobility, self-care, cognitive functioning, coordination, and daily living skills.
• Assess and recommend adaptive equipment, assistive technology, and environmental modifications.
• Educate patients, families, and caregivers on treatment strategies and home exercise programs.
• Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and other healthcare professionals.
• Maintain accurate and timely documentation in compliance with state, federal, Medicare, and facility regulations.
• Monitor patient progress and modify treatment plans as appropriate.
• Participate in care planning meetings and interdisciplinary team discussions.

Position Overview

The Occupational Therapist will evaluate and treat patients experiencing physical, cognitive, developmental, or psychosocial limitations that impact their ability to perform activities of daily living. Working collaboratively with interdisciplinary healthcare teams, the OT will develop and implement treatment plans designed to maximize patient independence and functional outcomes across SNF and home health environments.
