Job summary

MPH Care Services in Iowa seeks a licensed Physical Therapist to provide professional services within home health care, collaborating with primary care physicians and other health team members.

You will assess patients, develop and implement plans of care, conduct home safety evaluations, document visits within required timelines, and communicate progress with patients, families, and care teams.

Qualifications

• Successful completion of CAPTE-approved PT program or equivalent.
• A current, active license to practice Physical Therapy in Iowa.
• A minimum of two years’ experience as a Physical Therapist.
• Criminal background check & Current CPR certification.
• If trained outside the United States before Jan 1, 2008, meets World Confederation for PT requirements.

Responsibilities

• Receives intake referral information and prepares PT paperwork/tools for visits.
• Obtains medical history from patient, family, or significant others.
• Performs PT assessment and documents findings.
• Performs home safety check and environmental assessment.
• Develops and submits a Plan of Care (POC) for physician approval and updates every 30 days.
• Reassesses patient function and determines medical necessity for other services.
• Evaluates ADL/iADL abilities and supervises Home Health Aide when needed.
• Instructs patients and families on safe use of prosthetic/orthotic devices.
• Communicates with physician and other disciplines to optimize therapy.
• Submits OASIS assessments within 48 hours of visits.
• Participates in QA, staff meetings, and in-services.
• Travel to patient residences; may require transportation to various sites.
